Voluntary Benefits
For added protection, you can supplement your benefits with these voluntary plans from Unum.
Supplemental Hospital Insurance
This coverage complements your medical plan: it pays a lump sum you can use toward costs associated with a covered hospital stay. It also provides funds for out-of-pocket expenses your medical plan may not cover, such as coinsurance, copays and deductibles.
Group Critical Illness Insurance
This coverage helps protect your finances from the expense of a serious health event, such as a stroke or heart attack. Cancer coverage is also available. Lump sum benefits are paid directly to you at the time a covered condition is diagnosed. You may also elect family coverage, which includes coverage for specific childhood conditions such as Cystic Fibrosis and Down Syndrome.
You can use this coverage more than once. Even if you receive a payment for one illness, you’re still eligible to receive payment for other covered conditions. You can also receive up to 100% of your coverage amount for the reoccurrence of any critical illness, with the exception of skin cancer. Diagnoses must be 180 days apart or the conditions can’t be related to each other.
Group Accident Insurance
Unum Group Accident insurance pays benefits based on the injury you receive and the treatment you need, including emergency room care and related surgery. The benefit can help offset the out‐of‐pocket expenses that medical insurance may not pay, including deductibles and copays. Family coverage is available.
Individual Whole Life Insurance
Unum Individual Whole Life Insurance* offers a death benefit to your beneficiaries but it can also build cash value you can use while you are living. Later in life, you can use this cash value to buy a smaller, “paid up” policy with no more premiums due. You may choose coverage for yourself, your spouse and children.
If you are diagnosed with a terminal illness with a life expectancy of one year or less, you can request up to 100% of your benefit amount to a maximum of $150,000.
Note: The rates for this benefit may change on a class basis. Any accelerated death benefit payments will reduce the amount the policy pays upon the covered person’s death.
* The policy or provisions may vary or be unavailable in some states. The policy has exclusions and limitations which may affect any benefits payable. See the actual policy or your Unum representative for specific provisions and details of availability.
