Voluntary Life and AD&D Insurance
You can supplement the coverage from Tidelands Health by purchasing optional life and AD&D insurance.
Voluntary Life
You can buy additional life insurance coverage through The Hartford at group rates:
- For yourself: Elect coverage in $10,000 increments up to a maximum of $500,000 or 5x annual salary, whichever is less
- For your spouse: Elect coverage in $5,000 increments up to a maximum of $500,000 (amount elected cannot exceed team member amount)
- For your child(ren): Elect coverage in $2,000 increments up to $10,000 for each dependent child. For children between the ages of live birth and six months, the maximum payable benefit is $1,000.
Voluntary AD&D Insurance
You can buy additional AD&D insurance coverage through The Hartford at group rates:
- For yourself: Elect coverage in $10,000 increments up to a maximum of $500,000 or 5x annual salary, whichever is less
- For your spouse: Elect coverage in $5,000 increments up to a maximum of $500,000 (amount elected cannot exceed team member amount)
- For your child(ren): Elect coverage in $2,000 increments up to $10,000 for each dependent child. For children between the ages of live birth and six months, the maximum payable benefit is $1,000.
Evidence of Insurability
You must provide Evidence of Insurability (EOI) to The Hartford if you elect supplemental life coverage that is more than $150,000 for yourself or more than $50,000 for your spouse. If you previously waived coverage, you may elect $10,000 and spouses may elect $5,000 with no health questions during open enrollment.
Disability
Tidelands Health provides two types of disability coverage through The Hartford that pay benefits if you are unable to work due to illness or injury. Coverage is provided at no cost to all part- and full-time team members after 90 days of employment. Benefits begin after seven days of disability and continue for as long as you remain disabled.
Short Term Disability — STD
This coverage pays a weekly benefit that replaces 50% of your weekly base pay, to a maximum weekly benefit of $1,250.
Long Term Disability — LTD
LTD benefits begin after 180 days of disability and replace 50% of your basic monthly earnings, to a maximum monthly benefit of $5,000. Benefits may be reduced by deductible sources of income and disability earnings.
Additional Benefits for Directors, APP, CRNA and Physicians
Upon approval of disability claim by The Hartford and after the initial seven-day elimination period, you will receive salary continuation for up to eight weeks. Then any additional benefits would be paid under the Short Term Disability plan at 50% of weekly base pay, to a maximum benefit of $1,250 per week.
